The Community Care Social Worker/Case Manager works Monday through Friday with no call and no weekends. The role involves working in a multi-disciplinary approach to plan, coordinate, monitor, and supervise the provision of services to consumers enrolled in the Home and Community Based Services Program. The candidate must demonstrate the ability to deal with pressure to meet deadlines, be accurate, and handle constantly changing situations, as well as handle stressful situations and conflict. The position requires performing all job duties as assigned and adhering to all licensing board or agency requirements and Benefis Health System policies and procedures. Education and experience requirements include a Bachelor’s degree in social work or related fields, three years of social work experience in a health care setting preferred, knowledge of case management methods, diagnostic and crisis intervention skills, issues and needs of long-term care consumers, budget management, and understanding of human behavior, disabilities, and the aging process.
