About the job Care Coordinator


The Care Coordinator is responsible for facilitating patient-centered care through the coordination of clinical and non-clinical services in a remote environment. This role ensures continuity of care, supports patient engagement, and assists individuals in navigating healthcare services and resources. The Care Coordinator collaborates with healthcare providers, payers, and interdisciplinary care teams to improve patient outcomes, reduce care gaps, and promote efficient utilization of healthcare services.

Key Responsibilities:

Coordinate care activities for assigned patients across providers, services, and care settings

Serve as a primary point of contact for patients, families, and caregivers

Support care transitions, including hospital discharges and follow-up appointments

Assist patients with scheduling appointments, referrals, and diagnostic services

Educate patients on care plans, treatment adherence, and available resources

Monitor patient progress and identify barriers to care or social determinants impacting outcomes

Document care coordination activities accurately in electronic health records (EHR) or care management systems

Collaborate with physicians, nurses, social workers, and other care team members

Ensure compliance with HIPAA, organizational policies, and regulatory requirements

Participate in quality improvement initiatives, audits, and performance reporting

Required Qualifications:

Bachelors degree in Nursing, Social Work, Public Health, Health Administration, or a related field (or equivalent experience)

Minimum of 3-5 years of experience in care coordination, case management, or healthcare support roles

Strong knowledge of healthcare systems, insurance processes, and care transitions

Excellent communication, organizational, and problem-solving skills

Ability to work independently in a fully remote environment

Proficiency with EHR systems and standard office software

Reliable internet connection and private remote workspace

Preferred Qualifications:

Clinical licensure or certification (e.g., RN, LPN, CCM, CPHQ)

Experience in telehealth, population health, or value-based care models

Familiarity with Medicare, Medicaid, and managed care programs

Experience working with high-risk or chronic care populations

Annual Compensation:

Salary Range: $65,000 - $90,000 per year, based on experience and credentials.
